Welcome to the application form to participate in the EXTRA: Executive Training Program (EXTRA). We look forward to your participation in Cohort 19 of the program and to you becoming part of a pan-Canadian network of organizations and leaders that have benefited from it.
EXTRA is a team-based, bilingual, pan-Canadian leadership development Fellowship. EXTRA is not only an investment in the individual participant, it also builds leadership and organizational capacity for participating teams. Fellowship teams receive guidance from expert faculty and coaches and engage in peer-to-peer learning and networking with colleagues across Canada. The program’s curriculum continually adapts to the reality of today’s healthcare and social services environment.

PRE-APPLICATION COACHING CALL
Before applying to EXTRA, your team is strongly encouraged to request a free 30-minute coaching call, which will include a coach from EXTRA and a staff member from Healthcare Excellence Canada (HEC). This call will provide you and your executive sponsor with guidance and information on the following:
- Assembling the team
- Support provided
- Improvement project scope
- Improvement project framework
- Alignment with organizational strategic priorities
- Planning engagement of partners (patients, caregivers and community members)

SUBMITTING YOUR APPLICATION
To access the application form, you will first need to create an account. This account will allow you to view, download, draft, save and submit your application. All information entered is automatically saved; you may leave and return to the form to pick up where you left off. There is no commitment to complete or submit your application when you create an applicant account.
The application form is divided into five sections:
- The Organization(s)
- EXTRA Fellowship Team
- Improvement Project (IP)
- IP Implementation & Engagement Strategy
- Other Information
Detailed instructions are provided. You will be able to add collaborators (executive sponsor, proposed members of the Team, assistants, etc.) who may view and edit the application form.
The deadline for submission of your EXTRA application is July 3, 2024.
You must:
- identify a primary contact– this person will contact HEC with any question regarding the application.
- include an organizational chart of each sponsoring organization, including the names of the EXTRA Team members.
- build a team of three to five senior leaders, each having a significant scope of responsibility and influence within their organization.
- declare any potential conflict of interest as per HEC’s Conflict of Interest Policy.
- align the improvement project with the strategic priorities of the sponsoring organization(s).

NEXT STEPS
HEC will review all applications submitted to the requirements are met and will communicate with the primary contact if needed. An expert panel will then review the applications in detail, and will make recommendations to HEC for the selection of the participating teams. Any feedback will be shared with the applicants, whether accepted into the program or not.
Applicants will be notified of decisions in August 2024.
